Pantoprazole
Indications/Uses
Treatment of GERD, PUD. Prophylaxis for NSAID-associated ulceration. Treatment of pathological hypersecretory states (Zollinger-Ellison syndrome).
Dosage/Direction for Use
GERD 20-40 mg once daily for 4 wk, increased to 8 wk if necessary. Maintenance therapy: 20-40 mg daily. For recurring symptoms, an on-demand of 20 mg daily may be given. PUD 40 mg once daily. Duration of treatment: 2-4 wk for duodenal ulceration or 4-8 wk for benign gastric ulceration. Eradication of H. pylori Pantoprazole may be combined w/ 2 antibacterials in a 1-wk triple therapy regimen. Effective regimens: Pantoprazole 40 mg bd + clarithromycin 500 mg bd + amoxicillin 1 g bd, or combined w/ clarithromycin 250 mg bd + metronidazole 400 mg bd. Prophylaxis for NSAID-associated ulceration 20 mg daily. Treatment of pathological hypersecretory states (Zollinger-Ellison syndrome) Initially 80 mg daily, adjusted as required. Doses up to 240 mg daily have been used. Daily doses >80 mg should be given in 2 divided doses. Renal impairment Max of 40 mg daily.
Administration
May be taken with or without food: Swallow whole, do not chew/crush.
Contraindications
Hypersensitivity to pantoprazole or other substituted benzimidazoles (eg, esomeprazole, lansoprazole, omeprazole, rabeprazole ).
Special Precautions
Possibility of malignant GI ulcer should be excluded prior treatment, as pantoprazole may mask the symptoms & thus delay diagnosis. Mild, transient elevations of serum ALT (SGPT). Malabsorption of cyanocobalamin. Slightly increased risk of GI infections eg, Salmonella & Campylobacter & possibly C. difficile. Long-term (a year or longer) & multiple daily dose PPI therapy may be associated w/ an increased risk for osteoporosis-related fractures of the hip, wrist or spine. Patients should not drive or operate machines. Pregnancy & lactation. Childn <18 yr.
Adverse Reactions
Uncommon: Sleep disorders, headache, dizziness, taste disorders, diarrhoea, nausea/vomiting, abdominal distension & bloating, constipation, dry mouth, abdominal pain & discomfort, liver enzymes increased, rash/exanthema/eruption, pruritus, asthenia, fatigue & malaise.
Drug Interactions
Increased or decreased drug absorption at increased gastric pH values w/ gastric pH-dependent drugs (eg, ampicillin esters, Fe salts, ketoconazole). Potential increased INR & prothrombin time when used concomitantly w/ warfarin. Potential delayed absorption & decreased bioavailability w/ sucralfate. Possibility of methotrexate toxicities.
